It has been our constant
endeavour to bring the "better ways of buying" to
our valued customers and that is how we started the initiative of
"Any Time Medicine Card" (ATM CARD) - Customer
privilege card.
ATM card entitles our customers to
avail a Health Voucher worth Rs.500/- or a Free Health Insurance
worth Rs.20000/- on purchase of Medicines or general Healthcare
products worth Rs.6000/- from any of the Apollo pharmacies in the
concerned state with in a period of One Year.
Any customer purchasing from Apollo Pharmacy is eligible to take
the ATM loyalty card and it is absolutely free of cost.
Option 1: Health Voucher
On finishing the purchase target of Rs.6000/- with in a period of
one year the customers can avail Discount worth Rs.500/- on
purchase of medicines / general healthcare products at Apollo
Pharmacies or on any OP Treatment or Investigations at Apollo Group
of Hospitals in the concerned state. The Health Voucher is valid
for a period of two years from the date of issue.
Option 2: Free Health Insurance
Once the purchase target touches Rs.6000/- with in a period of one
year the customers are gifted with a Free Health Insurance of worth
Rs.20,000/-. The policy is valid for a period of one year only.
The ATM card is operational in states such as - Andhra Pradesh,
Tamilnadu, Karnataka, Delhi & NCR, Goa, Punjab, Gujarat,
Rajasthan & Maharashtra. The ATM card is valid only in the
concerned state.
However the benefits in ATM (Any Time Medicine) card slightly vary
from State to State.

The voucher can be availed

The Health Voucher can be
availed on purchases at Apollo pharmacies and this is
applicable to the ATM customers of the pharmacies in the
state of Andhra Pradesh, Tamilnadu, Karnataka, Delhi &
NCR, Goa, Punjab, Gujarat, Rajasthan & Maharashtra. |

The Health Voucher can be
availed on OP Treatment / Investigations at Apollo Group of
Hospitals (at select Apollo Hospitals) and is applicable
only to the ATM Card customers of Apollo Pharmacies in the
state of Andhra Pradesh, Tamilnadu, Karnataka, Goa, &
Maharashtra. |

The Health Voucher is valid
for a period of two years only. |

|
The Health Voucher has to be
handed over to the billing department of Apollo Group of
Hospitals / Apollo Pharmacies to avail the discount
facility. |

|
The Health Voucher is valid
for one time use only. |
|
|
|
The Health Voucher amount will
not be added into the ATM card Purchases. |
|
|
|
The processing time for
getting Health voucher is 7 working days from the time of
submission of relevant documents. |

|
|
Apollo Pharmacy reserves the
rights in case of any disputes. |
|
|
|
This offer should not be
combined with any other promotion offer in Apollo Pharmacy. |

Pre-existing is not covered.
Standard Mediclaim policy exclusions apply. |

Policy valid only for the age
group of 5 years to 65 years only. |

Only individuals are covered

The Insurance cover will be

accept any responsibility of the actions and decisions of
the Insurance provider that is National Insurance Company. |

The Policy is valid at Apollo
Group of Hospitals and some of the network hospitals of TPA. |

The processing time for
getting the policy card is 20-30 days from the time of
submission of relevant documents. |
