Information for patients
Drug Information This medication is used to make the urine less acidic. Urine that is less acidic helps the kidneys get rid of uric acid, helping to prevent gout and certain types of kidney stones (urate). This medication can also prevent and treat certain metabolic problems (acidosis) caused by kidney disease. Citric acid and citrate salts (which contain potassium and sodium) belong to a class of drugs known as urinary alkalinizers.
Drug Alert
Alert Sodium citrate should be used with caution in: the elderly or weak (debilitated) patients. It should not be used in: patients with inflammatory bowel disease (e.g. ulcerative colitis, a disease of the lower intestine [colon] characterised by open sores [ulcers] with symptoms such as frequent diarrhoea mixed with blood), patients with acute gastrointestinal conditions.

Possible Side Effects
General Information Nausea, vomiting, diarrhea, and stomach pain may occur. Mixing the medication with water or juice, taking it after meals, and drinking more fluids will help prevent these side effects.
Common Drug Side Effects swelling, tingling, or numbness in your hands or feet; muscle twitching or pain, leg pain or cramps; unusual weakness, rapid and shallow breathing, fast or slow heart rate, dizziness, confusion, or mood changes; feeling restless, nervous, or irritable; black, bloody, or tarry stools; severe or ongoing diarrhea; or seizure (convulsions).
Rare Drug Side Effects nausea, or vomiting, stomach pain; mild or occasional diarrhea; or mild stomach pain.

Before Consuming the Medicine
Avoid Drug blood disease, heart or blood vessel disease (may cause more serious conditions to develop), enlarged prostate, urinary tract blockage or difficult urination (phenothiazine-derivative antihistamines may cause urinary problems to become worse), epilepsy (phenothiazine-derivative antihistamines, especially promethazine given by injection, may increase the chance of seizures), glaucoma (may cause a slight increase in inner eye pressure that may worsen the condition, jaundice (phenothiazine-derivative antihistamines may make the condition worse), liver disease (phenothiazine-derivative antihistamines may build up in the body, which may increase the chance of side effects such as muscle spasms), Reye's syndrome (phenothiazine-derivative antihistamines, especially promethazine given by injection, may increase the chance of uncontrolled movements).
Drug Special Care
Drug Drug Interactions Erythromycin and ketoconazole increase the plasma levels of fexofenadine two- to threefold without influencing the QT interval. The reason for this effect is not well understood and could be caused by increased absorption or reduced gastrointestinal or biliary secretion.[7][12] Antacids containing aluminium or magnesium reduce the absorption of fexofenadine.[7] Fexofenadine is not to be taken with fruit juices as this may decrease the absorption of the drug.[13] Grapefruit juices can significantly reduce plasma concentration of fexofenadine. While St. John's Wort (hypericum perforatum) and its preparations do interact with fexofenadine, since fexofenadine is a P-glycoprotein substrate and hypericum is an inducer, this interaction is deemed "unlikely to cause problems" - see St John's Wort Interactions table

Excess Drug Consumption Reports of fexofenadine overdose are infrequent, and because of this, the effects are not well established. No deaths occurred in testing on mice, at 5000 mg/kg body weight, which is one-hundred and ten times the maximum recommended dose for an adult human. Further research shows no deaths in rats at the same concentration, which equates four hundred times the recommended dose in an adult human. Research on humans ranges from a single 800 mg dose, to a twice-daily 690 mg dose for a month, with no clinically significant adverse effects, when compared to a placebo.
