Alfacal 0.25 mcg Capsule is used to treat low blood calcium and phosphate levels. It effectively treats Vitamin D deficiency, renal osteodystrophy (changes in bone caused by kidney failure), hyperparathyroidism (parathyroid glands make high levels of calcium in the body), hypoparathyroidism (parathyroid glands make low levels of calcium in the body), hypocalcaemia (low levels of calcium), and rickets or osteomalacia (softening or deforming of bones due to lack of calcium). It contains Alfacalcidol, which is converted into the active form of vitamin D3 (calcitriol) and works by strengthening the bones by increasing the calcium and phosphate levels in the blood. In some cases, it may cause side effects such as weakness, fatigue, sleepiness, headache, loss of appetite, dry mouth, a metallic taste, nausea, and vomiting.

I was diagnosed with intrahepatic cholestasis of pregnancy (IHCP) when I was 24 weeks pregnant. I then started taking medication for my liver enzymes. At 36 weeks, during my third trimester, my alkaline phosphatase level increased to 400. Can this affect my baby’s lung development? Is it possible for the baby’s two lungs to mature differently because of this?

A rise in alkaline phosphatase in IHCP (a temporary liver condition that causes itching without a rash due to bile acid buildup in late pregnancy) is common and usually does not directly affect the baby’s lung development. Fetal lung maturity mainly depends on gestational age, and by 36 weeks, the lungs are generally well developed. It is very unlikely for the two lungs to mature differently because of this condition. However, close monitoring by your doctor is important to ensure your baby’s overall well-being.

I am 28 weeks pregnant and have an elevated alkaline phosphatase level of 138. Does this pose any risk to my baby or the delivery?

Alkaline phosphatase levels naturally increase during the third trimester because the placenta produces this enzyme as your baby grows. A level of 138 is typically considered normal during pregnancy and does not usually pose any risk to your baby or the delivery. However, your doctor may still monitor your liver function and perform other tests to rule out any liver or bile duct issues.

My GGT and ALP levels are high. What are the possible reasons for this, and how should these results be interpreted?

High GGT and ALP levels may occur due to liver inflammation, blocked bile ducts, gallbladder problems, fatty liver, alcohol consumption, certain medications, or bone disorders. Your doctor will interpret these results alongside your other tests and symptoms and may recommend further investigations, such as an ultrasound or bone scans, to determine the exact cause and guide appropriate management.

My alkaline phosphatase (ALP) level is high at 222 (normal range 45-129). My other liver function tests are normal. Six months ago, my ALP was around 150, and it appears to be increasing. Could you please advise what this might indicate and whether I should be concerned?

A gradual increase in ALP with normal liver tests may be caused by liver or bone problems, bile duct obstruction, or even normal growth or healing processes. Certain medications and vitamin deficiencies can also raise ALP. Since your levels have increased over the past six months, your doctor may recommend further tests to determine the exact cause and ensure there is no underlying serious condition.
