Not conceiving after one year of regular, unprotected intercourse may indicate reduced fertility, and this is quite common. It does not always mean there is a serious problem, but it is advisable for both partners to have a fertility check. In women, possible reasons include irregular or absent ovulation (release of a mature egg), hormonal imbalance, blocked fallopian tubes, endometriosis (a condition where tissue similar to the lining of the womb grows outside it), or thyroid problems. In men, a low sperm count or reduced sperm movement is a common cause. Lifestyle factors such as stress, being underweight or overweight, smoking, alcohol use, and poor sleep may also affect fertility in both partners. Doctors usually suggest simple tests such as semen analysis for the male partner, blood tests to check hormone levels in the woman, ultrasound scans, and tests to check if the fallopian tubes are open. Treatments such as fertility medicines, hormonal therapy, or assisted reproductive techniques may improve the chances of pregnancy. Having regular intercourse every 2 to 3 days, especially around the fertile period, eating a balanced diet, maintaining a healthy weight, and avoiding smoking and alcohol may also help. Consulting a gynaecologist or fertility specialist will help identify the exact cause and guide treatment.
