Tolvaptan

About Tolvaptan

Tolvaptan is a prescription oral tablet used to treat low levels of sodium in the blood, a condition known as hyponatremia. It is prescribed for individuals with heart failure, syndrome of inappropriate antidiuretic hormone (SIADH), or certain kidney conditions where sodium levels drop too low. This medication helps raise sodium concentrations to a safer, healthier range.

This medicine is typically taken once daily, and it is crucial to take it consistently at the same time each day. You can take it with or without food, but you must have access to water and drink when you feel thirsty. Avoid restricting your fluid intake while taking this medication unless specifically instructed otherwise by your doctor.

While taking Tolvaptan, you may experience common side effects such as dry mouth, increased thirst, or frequent urination. However, if you notice signs of liver damage—such as unusual fatigue, loss of appetite, right upper abdominal pain, or yellowing of your skin or eyes—you must contact your healthcare provider immediately.

Before starting this treatment, talk to your doctor if you have kidney disease, liver problems, or are unable to feel or respond to thirst. Do not take this if you cannot drink water, have very low blood volume, or are taking certain medications that interact with it.

This medication can interact with alcohol, which may worsen dehydration, and certain prescription medicines that affect how your body breaks down the drug. If you are pregnant, breastfeeding, or are an older adult, consult your doctor or pharmacist to discuss whether this treatment is safe for you.

Uses of Tolvaptan

Tolvaptan used in the treatment of hyponatremia (low sodium levels) and autosomal dominant polycystic kidney disease (ADPKD). The detailed uses of Tolvaptan are as follows: • Hyponatremia Treatment: Tolvaptan helps the body eliminate excess water and increase sodium levels in patients with heart failure or SIADH. • ADPKD Treatment: Tolvaptan slows the rate of kidney function decline in adults with rapidly progressing ADPKD, potentially delaying disease progression.

Medicinal Benefits

Tolvaptan helps manage conditions associated with low sodium levels and excess fluid retention by increasing the removal of free water while preserving sodium. It is also used to slow kidney function decline in certain patients with polycystic kidney disease. Key benefits include:

  • Corrects Low Sodium Levels: By increasing the excretion of excess water, Tolvaptan helps raise sodium levels in the blood, relieving symptoms such as confusion, weakness, and swelling associated with low sodium.
  • Reduces Fluid Retention: Tolvaptan helps remove excess water from the body without significant sodium loss, improving symptoms of fluid overload and reducing the need for strict fluid restriction.
  • Supports Kidney Health: In people with certain forms of polycystic kidney disease, Tolvaptan helps slow the decline in kidney function, supporting long-term kidney health and disease management.

Directions for Use

  • Tolvaptan can be taken with or without food. However, avoid taking it with grapefruit or its juice, as this may increase the effects of the medication.
  • It is usually taken once daily; however, your doctor may adjust the dosage based on your serum sodium and fluid levels.
  • Swallow Tolvaptan as a whole with a glass of water.
  • Do not crush, break, or chew it.

    Patients Concern

    Disease/Condition Glossary

Hyponatremia: Sodium is an important electrolyte that helps in maintaining water balance in and around the cells. It helps in proper nerve and muscle functioning and maintains stable blood pressure levels. Low levels of sodium in the blood are referred to as hyponatremia. Normal levels of sodium are 135-145 milliequivalent per litre. Hyponatremia occurs when the level is below 135mEq/L. Symptoms include weakness, tiredness, nausea, vomiting, headache, irritability, confusion and muscle spasms.

Autosomal Dominant Polycystic Kidney Disease (ADPKD): It is an inherited disorder in which cysts are developed in the kidney. These cysts are non-cancerous sacs that contain a water-like fluid which puts pressure on the surrounding tissues, leading to reduced kidney function. Symptoms include high blood pressure, swollen abdomen, and back or side pain.

    FAQs

Tolvaptan is used to treat hyponatremia (low levels of sodium) in patients with heart failure, liver disease and hormonal imbalances such as Syndrome of Inappropriate Antidiuretic Hormone (SIADH). Additionally, Tolvaptan is also used to treat autosomal dominant polycystic kidney disease (ADPKD).

Tolvaptan works by blocking the effect of vasopressin, a hormone which is involved in the formation of cysts in ADPKD patients. By blocking the effect of vasopressin, Tolvaptan slows down the development of cysts in the kidneys, reduces the symptoms of the disease, and increases urine production. Thereby increases serum sodium concentrations.

Dry mouth could be a side-effect of Tolvaptan. Limiting caffeine intake, avoiding smoking and mouthwashes containing alcohol, drinking water regularly and chewing sugar-free gum/candy might help in stimulating saliva and thereby prevents drying of the mouth.

Diarrhoea could be a side-effect of Tolvaptan. Drink plenty of fluid to prevent dehydration if you experience diarrhoea. Consult your doctor if you have severe diarrhoea or if you find blood in stools.

Tolvaptan may cause the liver to not work properly. Consult your doctor immediately if you experience nausea, vomiting, fever, loss of appetite, abdominal pain, tiredness, dark urine, jaundice, itching and flu-like symptoms.

Tolvaptan may cause water loss due to increased urine production. This might result in side-effects such as thirst, dry mouth and kidney problems. To prevent this, drink plenty of water regularly. Drink 1-2 glasses of water before going to bed even though you are not thirsty. Special care must be taken in patients with diseases that might reduce fluid intake.

Consult your doctor before taking Tolvaptan if you are diabetic as Tolvaptan might increase the blood sugar levels. Regular monitoring of blood glucose levels while taking Tolvaptan is advised.

You are advised to take Tolvaptan for the duration prescribed by the doctor. Tolvaptan should not be taken for more than 30 days.

Tolvaptan might increase the blood sugar levels. Consult the doctor if you are diabetic.

Do not take Tolvaptan if you are allergic to any of its components, if you have raised liver enzymes in the blood, very low blood volume, if there is no urine production, or if you cannot tell that you are thirsty.

Avoid consuming grapefruit or its juice while taking Tolvaptan as it might increase the effects of the medicine in the body.

Do not exceed the prescribed dose as it might cause an overdose. Taking an excess of Tolvaptan may cause excessive thirst, dizziness, excessive urination, or fainting. Consult the doctor if you experience these symptoms.

Tolvaptan is not recommended for patients with severe kidney disease. It should be used with caution in patients with mild to moderate kidney problems.

If you miss a dose of Tolvaptan take it as soon as you remember, However, if it is almost time for the scheduled dose, skip the missed dose and take the next dose at the scheduled time.

Tolvaptan is not recommended for use during pregnancy and breastfeeding.

Common side effects are dizziness, headache, dry mouth, fatigue, diarrhoea, thirst and increased urge to urinate. Talk to your doctor if you experience these side effects persistently.

Store Tolvaptan at room temperature, in a dry place. Keep out of sight and reach of children.

Treatment with Tolvaptan should be started and re-started in a hospital or in the presence of a healthcare professional to monitor the sodium levels in your blood closely. Too rapid correction of hyponatremia (low sodium levels) may cause osmotic demyelination syndrome (serious nerve damage). Consult the doctor if you notice symptoms of osmotic demyelination syndrome such as difficulty speaking, feeling that food or drinks are getting stuck in your throat, difficulty swallowing, mood changes, drowsiness, confusion, body movements that are difficult to control, weakness of the arms or legs, or seizures.

This medication must be started in a hospital to allow doctors to closely monitor your blood sodium levels. If sodium levels rise too rapidly, it can cause a serious and irreversible brain condition. Talk to your doctor if you have concerns about your hospital stay.

Watch for yellowing of your skin or eyes, dark urine, pale stools, severe fatigue, loss of appetite, or pain in the upper right side of your stomach. If you experience any of these symptoms, stop taking the medicine and call your doctor right away.

Generally, you should not restrict your fluid intake while taking this medication. You must drink water whenever you feel thirsty to avoid severe dehydration. Talk to your doctor for specific fluid guidelines tailored to your health condition.

You must not take Tolvaptan if you are unable to sense thirst or cannot drink water. Doing so can lead to life-threatening dehydration and rapid changes in blood chemistry. Talk to your doctor immediately if your ability to drink fluids changes.

This medication is generally not recommended during pregnancy unless the benefits clearly outweigh the potential risks to the unborn baby. If you are pregnant or planning to become pregnant, talk to your doctor to discuss safer alternatives.

If sodium levels rise too fast, it can cause osmotic demyelination syndrome, a severe neurological condition that affects speaking, swallowing, and movement. This is why close medical supervision and blood tests are mandatory during initial treatment. Talk to your doctor for more information.
