I have been passing stool 5 to 6 times daily for the past 2 months, and medicines have not helped much. What could be the reason, and what should I do next?

I have had a fissure for the past 3 months that keeps coming back, and even after taking lactulose and Darolac IBS, there is no improvement. What should I do?
