Persistent itching in the testicular area is commonly caused by a fungal infection (jock itch), sweating, poor ventilation, or sometimes irritation from soaps or tight clothing, and since anti-itch creams give only temporary relief, they may not be treating the root cause. The proper approach is to keep the area clean and completely dry, wear loose cotton underwear, avoid tight or synthetic clothes, and use a plain antifungal cream (instead of steroid-based creams) regularly for the full recommended duration, while avoiding sharing towels or clothing.
