Since standard treatments aren't helping, you may need a prescription-strength ointment to relax the sphincter muscle, such as generic nitroglycerin or diltiazem ointment. Applying generic lidocaine gel directly to the area can also provide temporary, immediate numbing relief, especially before bowel movements. Please ensure you are taking stool softeners to prevent straining. I highly recommend contacting your doctor or a specialist.

The white tissue is likely healing tissue or deeper skin layers, but a split incision needs to be evaluated by your surgeon or doctor to ensure it heals safely. I recommend temporarily stopping the Betadine (povidone-iodine) directly inside the wound, as strong antiseptics can sometimes delay the healing of delicate new tissue. While Betadine (povidone-iodine) is excellent for preventing infection initially, using it long-term can sometimes slow down the growth of healthy new skin. If your wound is dry, clean, and scabbed, it is generally safe to leave it uncovered. However, if it is still open, oozing, or rubbing against clothing, it is best to keep it clean, apply a plain protective ointment, and keep it covered. I advise against applying povidone-iodine (Betadine) ointment directly to the tear while you are already doing povidone-iodine sitz baths. It is much better to continue using your Crema gel, which is formulated to soothe the area, reduce swelling, and promote proper recovery.
