Hello, I have had rheumatoid arthritis for a very long time. I have been taking calcium, folitrax, HCQS, lefno, and Medrol for ten years. My ESR is consistently above. Please advise if there are any other therapy options. Regards

Methotrexate, hydroxychloroquine, leflunomide, and low-dose steroids like Medrol may not be able to completely reduce inflammation if you have long-term rheumatoid arthritis and a continuously high ESR. In order to better manage inflammation and prevent joint damage, rheumatologists typically reevaluate the treatment strategy in these situations. They may modify dosages, cut back on long-term steroid use if feasible, or add cutting-edge "biologic" or targeted therapy. Additionally, they look for hidden infections or anaemia as potential causes of elevated ESR. You should see a rheumatologist for a thorough evaluation and potential step-up therapy instead of sticking with the same medications since you have been on the same regimen for ten years.
