For melasma, the gold standard topical treatment is a prescription triple combination cream containing hydroquinone, tretinoin, and a mild steroid. Over-the-counter alternatives include azelaic acid, kojic acid, and vitamin C serums, which help fade pigmentation. Most importantly, daily use of a broad-spectrum sunscreen (SPF 30 or higher, ideally with iron oxides to block visible light) is mandatory, as sun exposure triggers melasma.
