With multiple Uterine fibroids in pregnancy, severe lower abdominal pain for a week can be due to fibroid-related degeneration (red degeneration) or pressure effects rather than a problem with Duphaston 10 mg, which is usually continued for hormonal support. Manage with rest, hydration, and a safe analgesic like Paracetamol, but avoid self-medicating beyond this. Since pain is significant and persistent, you should get a prompt obstetric review and repeat ultrasound to rule out complications and ensure fetal well-being.
