It is reassuring that your 2D Echo shows normal heart function, but persistent pain requires a review. This discomfort can sometimes be due to microvascular angina (small blood vessel issues) or spasms, which an Echo cannot detect. You should discuss alternative anti-anginal therapies with your doctor. Generic medications such as ranolazine (which improves myocardial oxygen use), ivabradine (to lower heart rate), nicorandil, or long-acting nitrates might be beneficial. Your cardiologist may also recommend a stress test or a coronary angiogram to evaluate the blood flow directly. Please do not adjust your current medications without medical supervision.
